It is subject to change, and because of the numerous variations in health insurance plans, we suggest you contact us to confirm eligibility. We ask that you bring your insurance card to each visit and show it to the receptionist when you check in. You are responsible for knowing what your co-pay is and for paying it at the time of service. Billing you for your co-pay increases our costs if you do not have insurance we offer self-pay rates
